US Quick Health LLC and its subsidiaries (“US Quick Health,” “we,” or “us”) own and operate websites located at www.usquickhealth.com, www.usquickhealth.net, www.usquickhealth.co, and www.usquickhealth.org (the “Websites”) and may now or in the future own and/or operate a US Quick Health mobile application (together, the “Platform”); your access to and use of the Platform, any portion of it, or anything connected with it, including its content (“Content”), any products or services made available through the Platform or otherwise by US Quick Health LLC, and any related website, software, or application owned or operated by US Quick Health LLC (collectively, including the Platform and the Content, the “Service”) are subject to this Privacy Policy unless expressly stated otherwise; capitalized terms not otherwise defined in this Privacy Policy have the meanings assigned to them in the US Quick Health LLC Terms and Conditions (“Terms and Conditions”).

We are committed to respecting and protecting the privacy of all users of the Service; this Privacy Policy has been created to explain how US Quick Health LLC collects, uses, and discloses information in connection with providing the Service.

By creating, registering, or logging into an account through the Service, or by otherwise accessing or using the Service, you acknowledge and agree to the most current version of this Privacy Policy; we may update this Privacy Policy from time to time to reflect material changes to the Service or to our handling of personal information, and to comply with applicable law; if changes are made to this Privacy Policy, the revised version will be posted and the “Last updated” date at the top of the Privacy Policy will be modified accordingly; where material changes occur, notice will be provided or consent obtained if required by law.

If you are using the Service on behalf of another individual, you represent and warrant that you are duly authorized to act on that individual’s behalf and that such individual has reviewed and accepts the practices and policies described in this Privacy Policy.

Certain telehealth services available through the Platform are provided by independent licensed medical providers operating through Foundation Health and its affiliated medical group, Beluga Health. US Quick Health LLC facilitates access to these providers but does not itself render medical services. The collection, use, and disclosure of information by Beluga Health are governed by its own Beluga Health Privacy Policy and Telehealth Consent.

Limitations on Use by Minors

The Service is intended for individuals who are at least eighteen (18) years of age, or such higher age as may be required by applicable state law in the jurisdiction where the Service is being accessed; individuals between the ages of thirteen (13) and eighteen (18) (or such higher age of majority) may only use the Service for the limited purpose of obtaining a medical consultation for the treatment of acne with topical skincare products, to the extent such products are made available, and only if a parent or legal guardian provides consent in accordance with the requirements set forth in the Terms and Conditions and the Service; the Service is not designed for, directed to, or intended to attract children under thirteen (13) years of age; if US Quick Health LLC becomes aware that it has collected personal information through the Platform from an individual under the age of thirteen (13), reasonable efforts will be made to stop further use of such personal information and to remove it from retrievable form.

If you are under sixteen (16) years of age, you (or your parent or legal guardian if you are under the age of thirteen (13)) may at any time request the removal of content or information about you that has been posted on the Platform; any such request (“Request for Removal of Minor Information”) must be submitted either (i) by mail to US Quick Health LLC, Attn: Privacy Officer, 7345 W Sand Lake Rd, Ste 210 – Office 2630, Orlando, FL 32819, with “Removal of Minor Information” clearly stated in the subject line, sent by U.S. Certified Mail, Return Receipt Requested, in order to allow for confirmation of mailing, delivery, and tracking; or (ii) by email to [email protected], with “Removal of Minor Information” in the subject line; each Request for Removal of Minor Information must clearly state: the nature of the request; the identity of the content or information to be removed; the location of the content or information on the Platform (for example, by providing the URL); confirmation that the request is related to the “Removal of Minor Information”; your name, street address, city, state, zip code, and email address; and whether you prefer to receive a response by mail or by email; requests submitted by telephone or facsimile will not be accepted; US Quick Health LLC is not responsible for failing to comply with any Request for Removal of Minor Information that is incomplete, mislabeled, or incorrectly submitted.

Please note that US Quick Health LLC is not required to erase or otherwise remove such content or information in certain circumstances, including but not limited to: when applicable international, federal, state, or local law, rule, or regulation requires that the content or information be maintained; when the content or information is retained on behalf of your Providers (as defined in our Terms and Conditions) as part of your electronic medical record; when the content or information was stored on or posted to the Platform by a third party other than you (including reposts or republication of your content by such third party) ; when the content or information has been anonymized such that you cannot be individually identified; when the request for removal does not comply with the procedures outlined above; or when you have received compensation or other consideration in exchange for providing the content or information.

The foregoing describes US Quick Health LLC’s voluntary practices concerning the collection of personal information through the Service from certain minors, and is not intended as an admission that US Quick Health LLC is subject to the Children’s Online Privacy Protection Act (COPPA), the Federal Trade Commission’s COPPA Rule(s), or any similar international, federal, state, or local laws, rules, or regulations.

Protected Information

When you create an account with US Quick Health LLC, you are establishing a direct customer relationship with US Quick Health LLC that allows you to access and use the functions of the Platform and the Service as a user; in doing so, you provide information to US Quick Health LLC, including but not limited to your name, email address, shipping address, phone number, and certain transactional details, which are handled in accordance with this Privacy Policy and applicable consumer privacy laws and are not classified as Protected Health Information (“PHI”).

When you use certain features of the Service, you may provide health or medical information that is protected under applicable federal or state laws and may constitute Protected Health Information (“PHI”); US Quick Health LLC operates as a technology platform and service provider and is not a “covered entity” or healthcare provider under the Health Insurance Portability and Accountability Act of 1996, Public Law 104-191, together with its regulations and amendments (“HIPAA”); US Quick Health LLC processes and stores PHI using HIPAA-compliant infrastructure and service providers, including cloud service providers and healthcare platform partners with whom we maintain appropriate Business Associate Agreements (“BAAs”), where required by law; further, any health or medical information you provide that is protected under applicable state law (together with PHI, “Protected Information”) will be used and disclosed only in accordance with such laws; information that does not constitute Protected Information under applicable laws may be used or disclosed in any manner permitted by this Privacy Policy; Protected Information does not include information that has been de-identified in compliance with applicable laws; the Medical Groups and Providers have adopted a Notice of Privacy Practices that explains how they use and disclose Protected Information, and by accessing or using the Service, you acknowledge receipt of the Notice of Privacy Practices from your Medical Group and Provider(s).

By accessing or using the Service, you understand thaat information submitted to US Quick Health LLC may be transmitted to licensed healthcare providers, medical groups, or healthcare platform partners solely as necessary to facilitate the services requested through the Platform and as permitted by applicable law and applicable BAAs; such providers independently collect, use, and process PHI in accordance with their own privacy policies, notices of privacy practices, and applicable federal and state laws; for clarity, information you provide to US Quick Health LLC when registering or setting up an account on the Platform, such as your name, date of birth, username, email address, shipping address, and phone number, does not constitute Protected Information.

Collection of Personal Information

The personal information collected by US Quick Health LLC depends on how you interact with the Service, the functions you use, and the choices you make. We collect information from multiple sources when you engage with the Service, including information you provide directly, information gathered automatically, information from third parties, and data that may be inferred or generated.

Information you provide directly may include:

• Name and contact details, such as your name, email address, phone number, billing address, and physical address;
• Demographic data, including gender, date of birth, and zip code;
• Social plug-in data, such as information shared via third-party websites, networks, platforms, servers, or applications (e.g., Facebook, Instagram, Twitter);

Information we obtain from third-party sources

We also collect personal information from external sources, which may include:

• Co-branding or marketing partners: entities with which we offer co-branded services or participate in joint promotional activities;

Cookies, Mobile IDs, and Similar Technologies

US Quick Health LLC uses cookies, web beacons, mobile analytics identifiers, advertising IDs, and other similar technologies in connection with the operation of its websites and online services; these tools assist in the collection of information, including usage data, identifiers, and device-related details.

These technologies may also include analytics tools that provide behavioral insights, such as heatmaps and session replay features, which help us understand how visitors interact with our websites and improve functionality, performance, and user experience.

What are cookies and similar technologies?

Cookies are small text files placed on your device by a website and stored in your browser; when your browser later connects to a web server in the same domain, the cookie can be read; cookies typically contain a string of numbers and letters that may uniquely identify your device and may also contain additional information; this enables a web server to recognize your browser over time each time it reconnects.

Mobile analytics and advertising IDs are identifiers generated by mobile device operating systems such as iOS and Android; applications can access and use these IDs in a manner similar to how websites access and use cookies; our mobile applications may include software that enables US Quick Health LLC and its third-party analytics and advertising partners to access such mobile identifiers.

Disclosure of Personal Information

We may disclose personal information with your consent or as necessary to complete transactions and provide the services you have requested or authorized; subject to the limitations described in the “Protected Information” section, each of the categories of personal information described in this Privacy Policy may be disclosed to the following types of third parties, as permitted or required by law:

• Legal and regulatory compliance: we may access, preserve, and disclose personal information where we believe it is required to comply with applicable law, regulations, or valid legal processes, including requests s from law enforcement agencies or government authorities.
• Security, safety, and rights protection: we may disclose information where necessary to (i) protect our users and others, for example to prevent fraud, spam, or to avert a risk of death or serious injury; (ii) maintain and secure the operation of the Service, including preventing or halting attacks on our systems or networks; and (iii) protect or enforce the legal rights, agreements, terms, and policies of US Quick Health LLC or the rights of others.

Access, portability, correction, and deletion

If you would like to review, obtain a copy of, download, correct, or request the deletion of personal information that US Quick Health LLC maintains about you, you may submit a request using the contact methods described at the end of this Privacy Policy; if certain personal information cannot be accessed, corrected, or removed directly through available self-service tools, we will assist you upon receiving such a request; in some U.S. states, you may also have the right to request details about the third parties with whom we share your personal information, and such requests can be made using the same contact channels.

Data sales

Browser or platform controls

Scope of this Privacy Policy

Please note that this Privacy Policy applies solely to personal information, which is distinct from medical information or protected health information; for details about how medical and protected health information is handled, and your rights with respect to that information, please review the Notice of Privacy Practices provided by the Medical Groups.

Data Retention

US Quick Health LLC retains personal information only for as long as it is necessary to deliver the services, complete the transactions you have requested, comply with legal requirements, resolve disputes, enforce agreements, and serve other lawful and legitimate business purposes; retention periods may differ depending on the type of data and the context in which it is collected; factors that influence the length of retention include user expectations or consent, the sensitivity of the data, the availability of automated tools that allow you to delete or manage data, and any applicable legal or contractual obligations.

Transactions

When you carry out a transaction through the Service (for example, purchasing products or services), you may be asked to provide certain information necessary to complete the transaction; this may include, without limitation, your billing address, shipping address, telephone number, and/or email address; by submitting such information, you grant US Quick Health LLC the irrevocable, royalty-free, worldwide, and perpetual right to use and disclose such information to third parties (such as banks or financial institutions) solely for the purpose of processing the transaction.

Payments for transactions conducted through the Service may be processed by bank wire transfer and/or by third-party payment processors, depending on the payment method made available at the time of purchase.

US Quick Health, LLC does not collect, process, store, or retain full credit card numbers, debit card numbers, bank account credentials, payment card security codes, or other complete payment instrument details. Payment information is processed directly by the applicable bank, financial institution, or third-party payment processor in accordance with its own terms, privacy policy, security practices, and applicable law.

If you make a payment by wire transfer, you may be required to transfer funds directly from your bank account to a designated bank account provided by US Quick Health, LLC or its authorized service providers. In such cases, US Quick Health, LLC may receive limited transaction-related information, such as the payer name, payment amount, payment date, transaction reference, and confirmation status, solely as necessary to confirm payment, issue refunds where applicable, maintain accounting records, prevent fraud, resolve disputes, and comply with legal or regulatory obligations.

By using the Service, you acknowledge that payment processing may involve banks, financial institutions, third-party payment processors, fraud prevention vendors, accounting providers, and other authorized service providers necessary to complete, verify, document, or reconcile a transaction. Any limited transaction-related information received by US Quick Health, LLC will be handled in accordance with this Privacy Policy and applicable federal and state privacy and consumer protection laws.

The relationship between US Quick Health LLC and the financial institutions involved in wire transfers is purely transactional and contractual; such institutions act as independent third parties and are not subject to the control or direction of US Quick Health LLC; accordingly, nothing in this Privacy Policy should be interpreted as creating a fiduciary, agency, partnership, joint venture, employment, or franchise relationship between US Quick Health LLC and any financial institution used for wire transfer processing.

The relationship between US Quick Health LLC and the financial institutions involved in wire transfers is purely transactional and contractual; such institutions act as independent third parties and are not subject to the control or direction of US Quick Health LLC; accordingly, nothing in this Privacy Policy should be interpreted as creating a fiduciary, agency, partnership, joint venture, employment, or franchise relationship between US Quick Health LLC and any financial institution used for wire transfer processing.

For example, when you use our online services, we allow advertising and analytics partners to collect certain information such as identifiers (IP addresses, cookie IDs, mobile IDs), activity data (browsing, clicks, app usage, non-product identifying transaction data), device data, and geolocation data through our sites and apps; however, we do not “sell” or “share” other categories of personal information.

once your opt-out is processed, we will no longer make such personal information available in ways considered a “sale” or “sharing” under the CCPA; however, we may still disclose some personal information to partners acting as our service providers in order to support advertising-related functions; please note that opting out will not retroactively undo prior “sales” or “sharing,” nor will it eliminate all forms of interest-based advertising. . 

We do not knowingly sell or share the personal information of individuals under the age of 16.

Right to Limit Use and Disclosure of Sensitive Personal Information

Before accepting such a request, we will require proof that the agent has been authorized to act for you, and in some cases, we may ask you to confirm your identity directly with us.

You also have the right not to be discriminated against for exercising any of your rights under the CCPA.

“Shine the Light” Law
In addition, California Civil Code Section 1798.83 (the “Shine the Light” law) allows California residents who have provided personal information to a business in the course of a personal, family, or household relationship to request information regarding whether the business has disclosed that personal information to third parties for their own direct marketing purposes.

Please note that US Quick Health LLC does not disclose personal information to third parties for their direct marketing purposes as defined by this law. California residents may request further information about our compliance with the Shine the Light law by emailing us at [email protected]. Be aware that businesses are only required to respond to one request per year per individual and are not obligated to respond to requests submitted through means other than the designated email address.

We take reasonable physical, technical, and administrative precautions to safeguard information against unauthorized access, use, disclosure, alteration, or destruction; however, it is your responsibility to keep your account password secure and confidential, and you are accountable for all activity conducted under your account; we strongly encourage you to use a robust password, never share it with others, and avoid reusing it across different sites or accounts; if you believe your account security has been compromised, you should notify us immediately using the instructions provided in the “Contacting Us” section below.

When using the Service, you may choose not to provide certain information; however, doing so may reduce the features available to you or prevent you from accessing the Service entirely; you may also opt out of receiving certain non-essential communications (such as newsletters or promotional offers) by emailing us with your preference; please note, even if you opt out of promotional messages, we may continue to send you service-related communications necessary for your use of the Service.

At present, US Quick Health LLC does not respond to web browser “Do Not Track” signals or similar mechanisms designed to opt out of cross-site information collection; should this change in the future, we will explain our practices in an updated version of this Privacy Policy.
