US Quick Health LLC and its subsidiaries (“US Quick Health,” “we,” or “us”) own and operate websites located at www.usquickhealth.com, www.usquickhealth.net, www.usquickhealth.co, and www.usquickhealth.org (the “Websites”) and may now or in the future own and/or operate a US Quick Health mobile application (together, the “Platform”); your access to and use of the Platform, any portion of it, or anything connected with it, including its content (“Content”), any products or services made available through the Platform or otherwise by US Quick Health LLC, and any related website, software, or application owned or operated by US Quick Health LLC (collectively, including the Platform and the Content, the “Service”) are subject to this Privacy Policy unless expressly stated otherwise; capitalized terms not otherwise defined in this Privacy Policy have the meanings assigned to them in the US Quick Health LLC Terms and Conditions (“Terms and Conditions”).

Certain telehealth services available through the Platform are provided by independent licensed medical providers operating through Foundation Health and its affiliated medical group, Beluga Health. US Quick Health LLC facilitates access to these providers but does not itself render medical services. The collection, use, and disclosure of information by Beluga Health are governed by its own Beluga Health Privacy Policy and Telehealth Consent.
Limitations on Use by Minors
The Service is intended for individuals who are at least eighteen (18) years of age, or such higher age as may be required by applicable state law in the jurisdiction where the Service is being accessed; individuals between the ages of thirteen (13) and eighteen (18) (or such higher age of majority) may only use the Service for the limited purpose of obtaining a medical consultation for the treatment of acne with topical skincare products, to the extent such products are made available, and only if a parent or legal guardian provides consent in accordance with the requirements set forth in the Terms and Conditions and the Service; the Service is not designed for, directed to, or intended to attract children under thirteen (13) years of age; if US Quick Health LLC becomes aware that it has collected personal information through the Platform from an individual under the age of thirteen (13), reasonable efforts will be made to stop further use of such personal information and to remove it from retrievable form.

Please note that US Quick Health LLC is not required to erase or otherwise remove such content or information in certain circumstances, including but not limited to: when applicable international, federal, state, or local law, rule, or regulation requires that the content or information be maintained; when the content or information is retained on behalf of your Providers (as defined in our Terms and Conditions) as part of your electronic medical record; when the content or information was stored on or posted to the Platform by a third party other than you (including reposts or republication of your content by such third party) ; when the content or information has been anonymized such that you cannot be individually identified; when the request for removal does not comply with the procedures outlined above; or when you have received compensation or other consideration in exchange for providing the content or information.
The foregoing describes US Quick Health LLC’s voluntary practices concerning the collection of personal information through the Service from certain minors, and is not intended as an admission that US Quick Health LLC is subject to the Children’s Online Privacy Protection Act (COPPA), the Federal Trade Commission’s COPPA Rule(s), or any similar international, federal, state, or local laws, rules, or regulations.

By accessing or using the Service, you understand thaat information submitted to US Quick Health LLC may be transmitted to licensed healthcare providers, medical groups, or healthcare platform partners solely as necessary to facilitate the services requested through the Platform and as permitted by applicable law and applicable BAAs; such providers independently collect, use, and process PHI in accordance with their own privacy policies, notices of privacy practices, and applicable federal and state laws; for clarity, information you provide to US Quick Health LLC when registering or setting up an account on the Platform, such as your name, date of birth, username, email address, shipping address, and phone number, does not constitute Protected Information.
