Unlesss you timely opt out in accordance with the instructions provided herein, all disputes between you and us — or between you and any affiliated medical group or provider — will be resolved through binding arbitration. By agreeing to these Terms, you waive your right to a jury trial and to participate in any class action, except as explicitly outlined below.
The Platform may include general health information (including educational articles, FAQs, and medication-related information) that is provided for informational and educational purposes only. Such information is not medical advice and is not a substitute for professional diagnosis, treatment, or individualized medical judgment.
Nothing on the Platform creates a doctor-patient relationship between you and US Quick Health, LLC. Any diagnosis, treatment, or prescription decisions are made solely by independent, U.S.-licensed healthcare professionals based on their clinical judgment, the information you provide (including intake questionnaire responses), and any additional information they may request.
Our internal Medical Board provides non-clinical, educational and scientific guidance to support the accuracy and responsible presentation of publicly available health-related information on the Platform. The Medical Board does not provide patient care and does not participate in prescribing or individual treatment decisions.
In case of a medical emergency, immediately seek in-person emergency care or call 911. The Service is not suitable for all health-related concerns or conditions.
These Terms may be updated from time to time as described herein.
About Us
US Quick Health LLC (“US Quick Health,” “we,” “our,” or “us”) is a Wyoming-based company that owns and operates a technology platform that facilitates access to telemedicine services provided by independent, U.S.-licensed healthcare professionals and affiliated medical groups, as well as access to pharmacy fulfillment and related services (collectively, the “Platform”). These Terms govern your access and use of the Platform, its content, and any products or services made available through it.
The Platform may include medical consultations, healthcare services, non-prescription products, or third-party offerings. These Terms apply to your use of the entire Service, including associated providers, content, and any communication or purchases made through the Platform.

These Terms outline key rights and responsibilities concerning your access and use of the Service. References to “you” or “your” include the individual using the Service, as well as parents or legal guardians acting on behalf of minors, where permitted. For details about use by minors, please refer to the “Limited Use and Availability” section.
The Service is not intended for anyone under the age of thirteen (13). Individuals below this age are strictly prohibited from creating accounts or using any part of the Service, even with parental consent. Acceptance of Terms and Conditions
Your use of the Service is subject to this Agreement and all applicable laws. If you do not agree with these Terms in their entirety, you are prohibited from accessing or using the Service, except to review the Terms. The Service is continually being improved and updated, and we reserve the right to change or remove any part of these Terms or the Service without prior notice. All changes are effective upon posting. Continued use after changes indicates your acceptance. .
Through our Service, individuals who register as users (“Users”) may access a variety of products and services offered either directly by US Quick Health LLC or through third-party licensed healthcare providers, partner pharmacies, laboratories, and vendors. Certain telehealth and prescription services available through the Platform are facilitated by Foundation Health Global, Inc., which coordinates care with licensed medical groups, including Beluga Health, P.A., and licensed pharmacies such as Healthwarehouse.com, Inc. (collectively, “Service Partners”). Prescription services are fulfilled via partner pharmacies, and laboratory tests may be processed by certified labs. The availability of such services may vary based on your location and applicable laws.
By using our Service, you authorize us to facilitate the transmission and fulfillment of your prescriptions with one or more licensed pharmacy partners within Foundation Health’s network. You also acknowledge that you may be connected with third-party medical groups and healthcare professionals (collectively, “Medical Providers”) who may offer medical and/or mental health consultations via the Platform.
The telehealth consultations available through the US Quick Health LLC platform are provided by independent licensed medical providers through Foundation Health and its affiliated medical group, Beluga Health. US Quick Health LLC facilitates access to these providers but does not itself render medical services. All professional medical services are subject to Beluga Health’s own Privacy Policy and Telehealth Consent.
By agreeing to these Terms, you confirm that any health-related services you receive from Medical Providers, labs, or pharmacies through the Platform are subject to these Terms, and these third parties — including Foundation Health Global, Inc., Beluga Health, P.A., and Healthwarehouse.com, Inc. — are considered third-party beneficiaries of this Agreement.
We do not practice medicine or mental healthcare. All clinical decisions and services are provided by independent Medical Providers, each of whom is solely responsible for their care and professional conduct. US Quick Health LLC does not control or interfere with clinical judgment or treatment decisions.
Medical Board (Educational Content Only). US Quick Health LLC maintains an internal Medical Board that provides non-clinical, educational and scientific guidance to help support the accuracy and reliability of general health-related information presented on the Platform. The Medical Board does not provide medical care, does not diagnose, and does not participate in prescribing or individual treatment decisions.
Clinical Decision-Making by Licensed Providers. All clinical decisions (including whether you are eligible for treatment and whether any medication is appropriate) are made solely by independent, U.S.-licensed healthcare professionals. Providers evaluate your medical intake questionnaire responses and other information you provide, and they may determine that you are not eligible for treatment or that an in-person evaluation is required. US Quick Health LLC does not control or interfere with a Provider’s clinical judgment, and we do not guarantee that you will receive any prescription.
By accepting this Agreement, you acknowledge and agree that US Quick Health LLC is not a healthcare provider, does not practice medicine or pharmacy, and does not interfere with the clinical judgment of any Medical Provider. All professional medical services accessed through the Platform are the sole responsibility of the licensed Medical Groups and Providers, including Beluga Health, P.A., coordinated through Foundation Health Global, Inc.
If you choose to engage with a Provider or Medical Group, you may receive communications from them — including diagnoses, follow-up instructions, or treatment plans — via the Service. You understand and accept that:
US Quick Health is not responsible for the security of any third-party communication systems (e.g., email, SMS) you use to receive these messages.
It is your responsibility to review and respond to such communications promptly.
As part of our efforts to ensure patient safety, we need to verify your phone number. By giving us your phone number and continuing, you agree that US Quick Health may send text messages to you to verify your phone number and for any other lawful pupose related to your US Quick Health account and your use of our services, including order confirmations, shipment notifications, and messages from your provider. Neither US Quick Health LLC, the Medical Groups, Foundation Health Global, Inc., Beluga Health, P.A., Healthwarehouse.com, Inc., nor any individual Provider shall be held liable for any harm, injury, or claim resulting from failure to read or act upon such communications.
Although you do not establish a doctor-patient relationship with US Quick Health LLC, by using the Service, you enter into a direct customer relationship with us solely for the purpose of accessing and facilitating telemedicine services through our platform.
US Quick Health LLC is not a medical provider and does not practice medicine or pharmacy. Instead, we provide a technology platform that connects users with independent, licensed medical providers and pharmacies — coordinated through Foundation Health Global, Inc. — who deliver clinical services and fulfill prescriptions in accordance with applicable federal and state regulations.
Any diagnosis, treatment, or prescription you receive through the Service is provided exclusively by third-party healthcare professionals or medical groups, such as Beluga Health, P.A., who bear full responsibility for your care, and prescriptions are dispensed solely by licensed pharmacies, including Healthwarehouse.com, Inc.
In using our Service, you may provide personal and health-related data, which we collect and process in accordance with our Privacy Policy.
For details about data handling, health information, and applicable regulations, please refer to the “Privacy Policy” and “Protected Health Information” sections of this Agreement.
US Quick Health LLC and the affiliated medical providers offering care through the platform do not participate in any federal or state healthcare programs, such as Medicare or Medicaid. Therefore, services and prescription products obtained through our platform are provided entirely on a self-pay basis.
By choosing to use our Service, you are voluntarily selecting to receive healthcare services and prescription products outside of any insurance reimbursement framework, including public health programs. You acknowledge and agree that:
Even if certain labs, pharmacies, or providers are independently enrolled in federal or state healthcare programs, the way in which services are offered through our platform — including asynchronous consultations and direct-to-consumer fulfillment — typically disqualifies them from being covered benefits under such programs.
US Quick Health, LLC is a telehealth coordination platform and does not operate as a pharmacy. US Quick Health, LLC does not directly sell, dispense, ship, or deliver prescription medications or medical products.
Where medically appropriate and permitted by applicable law, prescriptions issued by independent, licensed healthcare professionals may be fulfilled and shipped by licensed U.S. pharmacies or authorized fulfillment partners. Prescription fulfillment and delivery are subject to provider approval, prescription validity, pharmacy requirements, product availability, inventory, applicable federal and state laws, and carrier performance.
Shipping availability may vary by state, product, provider, pharmacy, and applicable law. Certain services, treatments, prescriptions, or products may not be available in all locations.
Any applicable shipping, handling, processing, pharmacy, or fulfillment-related charges are included in or disclosed as part of the total price displayed at checkout, unless otherwise stated. The total price shown at checkout represents the final amount payable for the selected products or services.
Estimated delivery timeframes, if provided, are estimates only and are not guaranteed. Delivery timing may depend on provider approval, prescription processing, pharmacy fulfillment, product availability, carrier performance, destination, weather conditions, holidays, and legal or regulatory requirements.
You are responsible for providing a complete, accurate, and legally eligible shipping address. Orders may be delayed, cancelled, or deemed undeliverable if the shipping information provided is incomplete, inaccurate, or not eligible under applicable law, provider requirements, pharmacy requirements, or carrier restrictions.

If, after completing a consultation through the Service, an independent, licensed healthcare provider determines that you are not medically eligible and does not approve the requested prescription, treatment, or related service, you will receive a full refund of the amount paid for the applicable order.
You are responsible for providing accurate, complete, current, and verifiable information. If you provide false, incomplete, inaccurate, unverifiable, inconsistent, or misleading information, or fail to respond to reasonable verification or compliance requests, US Quick Health, LLC may reject, cancel, suspend, delay, or refuse the applicable order or service. In such cases, any refund, partial refund, credit, or other resolution will be determined at our sole discretion, subject to applicable law and any costs, fees, services, or administrative work already incurred.
Once a licensed healthcare provider approves the requested prescription, treatment, or related service, the applicable order is no longer eligible for cancellation, return, or refund, except where required by applicable law. Prescription medications and healthcare-related products cannot be returned, cancelled, or refunded once approved, fulfilled, dispensed, or shipped by a licensed pharmacy or authorized fulfillment partner.
Eligible refunds, if any, will be issued to the original method of payment within a reasonable period of time, typically within 5–10 business days.

By proceeding with the use of the Service, you explicitly accept these financial terms and confirm your understanding that all services and products are offered on a direct-pay basis only.
If you believe you received an incorrect, damaged, defective, or otherwise improper medication or healthcare-related product, do not use the product and contact us immediately at [email protected]. Such issues will be reviewed on a case-by-case basis in coordination with the applicable licensed pharmacy or fulfillment partner.
Certain products available through the Platform require a valid prescription from a licensed medical provider. You will only be able to receive such prescription products after completing a consultation with an authorized Provider, who will determine whether the medication is clinically appropriate for you.
If a licensed Provider determines that a prescription medication is appropriate for you, they will issue a valid prescription in accordance with applicable laws and regulations. Prescription services available through the Platform are fulfilled by licensed third-party pharmacies and may involve the use of other independent service providers as required for order processing and delivery.
In most cases, your prescription will be processed, dispensed, and shipped directly to you by one of our affiliated pharmacies. Where permitted by law and supported by our platform, you may be offered the option to have your prescription filled at a pharmacy of your choice. However, this option is not guaranteed and is only available when expressly indicated through the Platform.
Prescriptions may also be transferred between authorized pharmacy partners within Foundation Health’s network without prior notice, as permitted by law, to ensure continuity of service and timely fulfillment.
Certain prescription products may not be available through our partner pharmacies and, where required by law or clinical necessity, must be filled by a local pharmacy as instructed by your Provider.
If you choose to have your prescription filled through one of our pharmacy partners, the medication will be dispensed and shipped to you by that licensed pharmacy. All associated costs — including medication price, pharmacy dispensing fees, and applicable service charges — will be clearly displayed as part of your order summary prior to checkout.
Please note that prescription products available through the Platform may not always be dispensed in child-resistant packaging.
All prescription products offered through the Platform constitute “Third-Party Goods and Services” as defined in these Terms and are subject to applicable federal and state regulations.
Certain products offered through the Platform may be available as part of an auto-renewing subscription program, in which you receive recurring shipments or ongoing access to services at regular intervals (e.g., monthly). If you opt into such a subscription, you authorize US Quick Health LLC to bill your payment method automatically and on a recurring basis, wit hout requiring further consent for each transaction.
Each subscription period will be billed in advance, and the amount charged will include the cost of the product or service, applicable service fees, shipping, and taxes. The details of your subscription, including frequency and pricing, will be disclosed to you at the time of your initial enrollment and may be updated from time to time.
You may cancel your subscription at any time by logging into your account or by contacting us at [email protected]. In order to avoid being billed for the next subscription period, your cancellation request must be submitted at least 48 hours prior to the next scheduled renewal date. Cancellations made after that time may not take effect until the following cycle.
We reserve the right to suspend or cancel a subscription at our discretion, including if a healthcare provider determines that continued use of the product or service is no longer medically appropriate. Additionally, subscriptions may be modified or discontinued due to changes in availability, applicable law, or business considerations.
All subscription products and services are provided subject to availability, and continued fulfillment depends on prescription validity, inventory, and compliance with applicable state and federal regulations.
The services offered through the Platform are intended only for use by individuals who are 18 years of age or older and reside in the United States. By using the Platform, you confirm that you meet these eligibility requirements.
Access to specific products, services, and licensed healthcare providers is limited to certain U.S. states, based on applicable medical licensing and telehealth laws. You may not access or use the Platform in any jurisdiction where our services are restricted, prohibited, or otherwise unavailable by law. When consulting with a licensed Provider, you represent and warrant that you are physically located in the same state as the shipping address provided in your account at the time of consultation.
By accessing or using the Platform, you agree to comply with all applicable federal, state, and local laws and to use the Service solely for lawful purposes. Our services are subject to state and federal regulations and may change from time to time to ensure ongoing compliance with applicable telehealth requirements.
Our services are not intended for use outside the United States, and we make no representations that the Platform or its content is appropriate or available in other locations. Any access from outside the U.S. is at your own initiative and risk, and you are responsible for compliance with local laws.
Certain products available through the Platform may be subject to additional age or use restrictions, depending on applicable state or federal law. Additionally, the Platform may not be suitable for all medical conditions or treatment needs. In some cases, your Provider may determine that an in-person examination, diagnostic test, or alternative form of care is required, and you may be referred for in-person treatment.
We may, at our sole discretion, suspend, restrict, or terminate your access to the Platform if we have reason to believe that you have violated these Terms, provided inaccurate information, or engaged in unauthorized activity. We also reserve the right to restrict or disable access to any user at any time, for any reason, without notice.
You understand and agree that the Platform, including all products, services, features, and content, is provided “as is” and “as available,” without warranty of uninterrupted availability. We may modify, suspend, or discontinue any part of the Platform at any time, including specific services, provider access, or prescription offerings, without prior notice or liability.
In addition, you may not export or re-export any prescription products, laboratory kits, or services obtained through the Platform in violation of any applicable laws or export control regulations.
Telehealth refers to the delivery of healthcare and mental health services through electronic communication methods, including information technologies and other digital tools, where the patient and the healthcare provider are not physically in the same location.
While telehealth can offer certain advantages — such as increased accessibility and convenience — it also carries potential risks. These may include limitations in clinical assessment due to the absence of physical examination, possible delays in care due to technical failures, or data security concerns. Telehealth services are not a substitute for in-person care in all situations, and may not be appropriate for every medical or mental health condition.
When creating an account or using the Platform to access telehealth services, you will be required to review and electronically agree to a separate Informed Consent for Telehealth Services (“Patient Consent”) provided by Beluga Health, P.A., the independent medical group delivering telehealth care through the Platform. This consent explains the nature, risks, and limitations of receiving medical services via telehealth and must be accepted before any telehealth consultation can occur.
You acknowledge and agree that US Quick Health LLC is an intended third-party beneficiary of the Patient Consent and may enforce its terms against you where applicable.
To access and use the services provided through the Platform, you are solely responsible for ensuring that you have and maintain, at your own cost, the necessary hardware, software, and internet connectivity. This may include a functioning computer or mobile device equipped with a camera, microphone, and a stable internet connection, consistent with any technical specifications required by the Platform.
You are also responsible for any associated costs, such as internet service charges, data usage fees, or mobile connectivity costs that you may incur while using the Platform.
Furthermore, you agree to provide accurate, complete, and truthful information in all forms, applications, or communications submitted to or through the Platform. Submitting false or misleading information may result in the denial of services, termination of your account, or other consequences as permitted under these Terms.
US Quick Health LLC reserves the right to modify or update the technical requirements or access configuration for the Platform at any time, including but not limited to necessary hardware, software, browser compatibility, or internet specifications, without prior notice.
US Quick Health LLC recognizes the importance of maintaining the privacy and confidentiality of your personal and health-related information. For details about how we collect, use, disclose, and protect your data, please refer to our Privacy Policy, which is incorporated into these Terms by reference.
When you create an account with US Quick Health LLC, you establish a direct customer relationship with us that enables you to access and use various features of the Platform. In the course of this relationship, you may provide personal information such as your name, email address, shipping address, and phone number. This type of information may be collected, used, and disclosed by us in accordance with our Privacy Policy and applicable privacy laws.
However, when you engage with specific services available through the Platform, you may also submit medical or health-related information that is protected under applicable federal or state laws. Such information may constitute Protected Health Information (“PHI”).
US Quick Health LLC operates as a technology platform and service provider and is not a healthcare provider. We do not make medical diagnoses, provide medical treatment, or prescribe medications. Licensed healthcare providers, pharmacies, laboratories, or medical groups affiliated with the Platform are independently responsible for all medical services and clinical decisions.
US Quick Health LLC processes and stores PHI using HIPAA-compliant infrastructure and service providers, including cloud service providers and healthcare platform partners with whom we maintain appropriate Business Associate Agreements (“BAAs”), where required by law. PHI is handled in a manner intended to comply with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and applicable state privacy laws, including the implementation of appropriate administrative, technical, and physical safeguards.
Additionally, any medical or health-related information governed by state-specific privacy laws (together with PHI, referred to here as “Protected Information”) will be collected, used, and disclosed in accordance with applicable legal requirements.
Information that has been de-identified in accordance with HIPAA or applicable law does not constitute Protected Information and is not subject to the same restrictions.
The affiliated medical groups and healthcare providers available through the Platform maintain their own Notices of Privacy Practices, which describe how they collect, use, and disclose Protected Information in accordance with applicable law. By using the Platform and receiving services from a healthcare provider, you acknowledge receipt of the applicable Notice of Privacy Practices from your provider or medical group.
Information that is not classified as Protected Information under federal or state law is handled in accordance with our Privacy Policy and applicable state consumer privacy laws.
Protected Health Information may be shared with licensed healthcare providers or medical groups affiliated with the Platform, including through healthcare platform partners, solely as necessary to facilitate the services requested through the Platform and as permitted by applicable law.
Healthcare providers and medical groups affiliated with the Platform independently collect, use, and process Protected Health Information in accordance with their own privacy policies, notices of privacy practices, and applicable federal and state laws, including HIPAA, and are solely responsible for their own compliance obligations.
By creating an account and using the Platform, you acknowledge that US Quick Health LLC is not a covered entity or healthcare provider under HIPAA and provides services solely as a technology platform and service provider.
To access the services offered through the Platform, you must register and create a personal user account. The services are only available to individuals who have successfully registered and have been granted login credentials, including a username and password.
The information you provide during the account registration process — including, but not limited to, your name, username, email address, shipping address, and phone number — is not considered Protected Health Information as defined in the section above on Protected Health Information. This personal data is instead governed by our Privacy Policy.
You agree to provide accurate, current, and complete information when creating your account and to promptly update such information as necessary. This applies to all information provided to US Quick Health LLC, our affiliated medical groups or providers, laboratories, or pharmacies. If any information you provide is found to be untrue, inaccurate, incomplete, or outdated — or if we have reasonable grounds to suspect so — we reserve the right to suspend or permanently terminate your account and access to the Platform.
You are solely responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account. You agree to:
We disclaim all liability for any loss or damage arising from your failure to comply with these obligations. You must not use any other person’s account at any time, nor allow others to use yours.
When creating an account, you will be required to set a secure password and may need to provide additional verification information. To help protect any personally identifiable information you transmit, we employ encryption technologies such as Secure Sockets Layer (SSL) or comparable measures to ensure data security during transmission.
We also implement internal safeguards to protect the user information we collect from unauthorized access. However, please be aware that the Platform operates through software, hardware, and network systems, any of which may, from time to time, require updates, maintenance, or may experience unexpected disruptions or security vulnerabilities beyond our control.
Moreover, individuals with access to your devices — such as your computer, smartphone, or tablet — may be able to view your account or access sensitive information, including medical records or communication. It is your responsibility to log out after each session and to secure physical access to your devices.
You agree to use the Platform with caution and sound judgment. You must not attempt to compromise or violate the security of the Platform in any way. Unauthorized access, tampering, reverse engineering, or other malicious actions may result in civil and/or criminal penalties. We reserve the right to investigate suspected violations and may cooperate with law enforcement authorities if illegal activity is suspected.
Except where limited by applicable privacy laws or protections for Protected Health Information, any content, feedback, questions, comments, suggestions, or other material that you submit to US Quick Health LLC through the Platform—whether by direct entry, forms, email, or any other method—will, to the fullest extent permitted by law, be treated as non-confidential and non-proprietary. By submitting such content (collectively, “Submissions”), and unless otherwise restricted by your account settings or applicable laws, you grant US Quick Health LLC a worldwide, perpetual, irrevocable, royalty-free, non-exclusive, fully sublicensable license to use, reproduce, adapt, publish, translate, distribute, publicly display, and modify the Submission in whole or in part for the purposes of providing, maintaining, and improving the Service.
You represent that you have obtained all necessary rights to grant this license and that any holder of moral or intellectual property rights in the Submission has waived such rights or granted you the authority to do so. You remain solely responsible for the content of all Submissions you provide. US Quick Health LLC may, at its sole discretion, edit, adapt, or otherwise modify the Submission for operational, promotional, or editorial purposes, and you waive any right to inspect or approve any use of the Submission. You further agree that you are not entitled to compensation, credit, or any other form of reimbursement in connection with your Submission.
In cases where your Submission contains information classified as Protected Health Information or subject to specific legal protections, US Quick Health LLC will limit its use and disclosure of such data in accordance with applicable law and its Privacy Policy.
The Platform, along with all content, data, design elements, and other materials made available through it (collectively, the “Service”), is protected under copyright, trademark, and other intellectual property laws of the United States and international jurisdictions.
Subject to your compliance with these Terms, US Quick Health LLC grants you a limited, revocable, non-transferable, and non-exclusive license to access and use the Service for your own personal and non-commercial use. Unless explicitly stated otherwise in a separate agreement, this license does not grant you any ownership or proprietary rights in the Service or its contents.
All rights, title, and interest in and to the Service — including, without limitation, all source code, graphics, artwork, product information, blog content, animations, audio, audiovisual components, design features, methods of operation, and documentation — are and shall remain the exclusive property of US Quick Health LLC and its licensors or suppliers.

The US Quick Health name, logo, and any related design marks, trade names, or service marks that appear on the Platform are trademarks or registered trademarks of US Quick Health LLC and may not be used without our prior written consent. All other trademarks, service marks, or brand names that appear in connection with the Platform belong to their respective owners and are used for identification purposes only.
You are strictly prohibited from using or attempting to use the Service for any unlawful, unauthorized, fraudulent, or malicious purpose. This includes any action that could damage, disable, disrupt, or otherwise impair the Service or any server, system, or network connected to the Service, or interfere with the use and enjoyment of the Service by others. You may not attempt to gain unauthorized access to other accounts, computer systems, or networks through methods such as hacking, password mining, or any other illegitimate means. Furthermore, you are not permitted to access or attempt to access any systems, data, or information not intended to be made available to you, nor may you obtain materials or information by any means not intentionally provided through the Service. Reverse engineering, disassembling, or decompiling any part of the technology on or associated with the Platform is prohibited. The Service may only be used for its intended business purpose.
In using the Service, you agree not to upload or transmit any content that is unlawful, harmful, threatening, abusive, harassing, defamatory, vulgar, obscene, libelous, or otherwise offensive or inappropriate, including but not limited to content that targets race, gender, sexuality, ethnicity, religion, or any other intrinsic characteristic, or that infringes on the rights of privacy or publicity of others. You may not impersonate any individual or entity, misrepresent your affiliation, or create false identities or duplicate accounts intended to mislead or deceive. You are prohibited from uploading or distributing any content that you do not have the legal right to share under intellectual property laws, contracts, or fiduciary obligations, including nondisclosure agreements. You must not upload files containing viruses, worms, Trojan horses, spyware, or other malicious code or software intended to damage, disrupt, or interfere with computer systems or data.
You may not delete or alter legal notices, proprietary labels, or author attributions on any content you upload or interact with. Using communication features in ways that disrupt availability for others — such as excessive use of capital letters or repeated messages — is not allowed. You are prohibited from sending unsolicited advertising, spam, chain letters, pyramid schemes, phishing messages, or other types of unauthorized solicitation. Your use of the Service must comply with all applicable local, state, national, and international laws. Content that infringes upon any patent, trademark, copyright, trade secret, or proprietary rights is not permitted. You may not delete or edit content posted by other users, manipulate the Service’s layout through framing or mirroring techniques, or attempt to bypass authentication or security mechanisms. Unauthorized registration or unsubscription of users to services, contests, or promotions without their consent is forbidden. Collecting or harvesting data about other users, including email addresses, without permission is not allowed. Furthermore, you may not use any automated or manual tools — such as bots, spiders, or scrapers — to access or reproduce content on the Platform, nor may you assist or enable others in engaging in any of these prohibited activities.
US Quick Health LLC reserves the right to take any lawful actions it deems appropriate in response to violations or suspected violations of these terms. This includes, but is not limited to, suspending or terminating your account and access to the Platform. We may also cooperate with law enforcement agencies or third parties in the investigation of any suspected criminal or civil wrongdoing.
Except as limited by our Privacy Policy or applicable law, we reserve the right at all times to disclose any information we deem necessary to comply with legal obligations, regulations, court orders, or government requests. We also reserve the right to remove, edit, or refuse to post any content or materials submitted to or available through the Platform, at our sole discretion.
Certain services and products offered through the Platform are provided by parties other than US Quick Health LLC, including but not limited to laboratories, pharmacies, independent medical groups, and healthcare providers (collectively referred to as “Third Parties”). In addition, US Quick Health LLC may offer access to or facilitate the purchase of certain goods, devices, or services that are manufactured, distributed, or sold by such Third Parties (“Third-Party Goods and Services”). Your use of these Third-Party Goods and Services, as well as any interactions, transactions, or communications with Third Parties, including but not limited to payments, delivery, warranties, and representations, are solely between you and the relevant Third Party. It is your responsibility to conduct any investigation you deem necessary or appropriate before engaging in any transaction, whether online or offline, involving any Third Party or Third-Party Goods and Services. You remain fully responsible for exercising caution, discretion, and sound judgment when using the Service and sharing any personal information.
You acknowledge and agree that US Quick Health LlC shall not be held responsible or liable for any loss, damage, claim, or other consequence resulting from your use of the Service, including any dealings with Third Parties or use of Third-Party Goods and Services. In the event of a dispute between you and any Third Party, or between you and another user or any other individual or entity, you understand and agree that US Quick Health LLC is under no obligation to intervene or become involved. You hereby release and agree to indemnify and hold harmless US Quick Health LLC, its affiliates, subsidiaries, and related companies, as well as their respective officers, directors, employees, contractors, agents, successors, and assigns, from any and all claims, demands, or damages of any kind or nature, whether known or unknown, suspected or unsuspected, disclosed or undisclosed, arising out of or related to any such disputes or to your use of the Service or any of its features.
If you are a resident of California, you expressly waive the application of California Civil Code Section 1542, which states: “A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party.”
You further understand that certain owners, shareholders, directors, officers, employees, agents, or contractors of US Quick Health LLC may have a financial interest in one or more Third Parties, and may derive benefit from your use of Third-Party services or the sale of Third-Party Goods and Services facilitated through the Platform.
US Quick Health LLC reserves the right to monitor the general use of the Service at any time and in any manner it deems appropriate. We may, at our sole discretion, review, screen, remove, or take other action with respect to any content or material submitted to or accessible through the Service if we believe that such material may be unlawful, may expose US Quick Health LLC to liability, may violate these Terms, or may otherwise be inconsistent with the intended purpose of the Service.
All products and services offered for purchase through the Platform are subject to availability, and US Quick Health LLC reserves the right to impose quantity limits on any order or to reject all or part of an order at any time and without prior notice. In the event of a pricing or listing error, we reserve the right to correct the error and revise your order accordingly, which may include charging the corrected price, or to cancel the order and refund any amount already paid. Prices for products and services are subject to change at any time without notice. You are responsible for paying any applicable sales, use, excise, customs duties, or other taxes, fees, or governmental charges (“Taxes”) associated with your purchase through the Platform. Where legally required, we will collect applicable Taxes and display an estimate of those Taxes at checkout. The actual amount charged may differ from the estimate due to factors such as changes in tax rates or differences in payment processor calculations. We do not collect Taxes in every jurisdiction, and in such cases you may be required to report and pay Taxes directly to your local authority.
Only valid and authorized payment methods accepted by us may be used to complete purchases through the Platform. By submitting an order, you represent and warrant that you are authorized to use the designated payment method and authorize us to charge that method for the total amount of your order, including all applicable Taxes, shipping, and handling fees. If your payment is declined, we may attempt to reprocess the charge until approved. Our third-party payment vendors may also request and receive updated payment information, such as new card numbers or expiration dates, directly from your card issuer. By using the Service, you consent to our use of such updated information to process future payments, including subscription charges, unless you opt out by contacting your card issuer. We are not responsible for any fees or charges imposed by your bank or card issuer. If a payment is reversed by your bank or issuer, we may charge your account again or seek payment through an alternative method, including invoicing.
If one or more products in your order are unavailable, you will only be charged for the items actually shipped, together with applicable Taxes and fees. In completing a purchase, you may be required to provide transaction-related details such as your payment card number and expiration date, billing and shipping addresses, phone number, and email address. By submitting this information, you grant US Quick Health LLC the right to transmit such details, as well as any updated information, to third-party vendors as necessary to complete the transaction and to facilitate future orders or subscription payments.
All monetary transactions on or through the Platform are processed through secure third-party payment applications integrated into the Service.
All monetary transactions on or through the Service currently occur via wire transfer (bank transfer) using the instructions provided to you during checkout. Transactions will only be processed once funds have been successfully received and confirmed by US Quick Health LLC. We reserve the right to add additional payment methods, including third-party payment processors, in the future.
You agree to pay all applicable shipping and handling charges disclosed at the time of purchase. We reserve the right to increase, decrease, add, or eliminate shipping and handling fees from time to time, provided that any changes will be disclosed to you before you complete your purchase. Any delivery dates or timeframes provided at checkout are estimates only and are not guaranteed. Unless otherwise stated in writing, the risk of loss or damage to products transfers to you upon delivery of the items to our designated carrier.
We reserve the right to address and resolve user concerns and issues on a case-by-case basis. Any resolution will be determined at our sole discretion, taking into account the specific circumstances of each matter.
US Quick Health LLC may suspend or terminate your access to the Service, including any features or components of the Platform, at any time and for any reason without prior notice. Termination may occur, for example, if you violate these Terms, if one or more of our agreements with affiliated medical groups, laboratories, or pharmacies ends, or if we choose to discontinue the Service in whole or in part.
The provisions of this Agreement relating to Service security, prohibited conduct, intellectual property, user submissions, disclaimers, limitations of liability, dispute resolution and arbitration, indemnification, and jurisdictional matters shall survive termination of your account, termination of these Terms, or the end of your relationship with US Quick Health LLC. You agree that if your right to access the Service is terminated under this Agreement, you will not attempt to access the Service again under your own name or under any other name, real or fictitious. You further agree that if you breach this restriction, you will indemnify and hold harmless US Quick Health LLC and its affiliates from any liability arising as a result.
Unless otherwise required by applicable law or expressly provided in our Privacy Policy, including any obligations to provide you with access to your medical records, US Quick Health LLC has no duty to return or provide to you, or to any third party acting on your behalf, any information, submissions, records, or content that you provided to us, that providers shared with us about you, or that we otherwise maintain in connection with your use of the Service, either before or after termination of your account.
All content and information made available through the Platform is provided by US Quick Health LLC for general informational purposes and as a convenience to users. Any reliance on such content or information is at the sole risk of the user.
For clarity, general health-related content on the Platform is informational only and does not constitute medical advice. You should not rely on Platform content as a substitute for consulting a qualified healthcare professional. Never disregard or delay seeking professional medical advice because of something you have read on the Platform.
The internal Medical Board’s role is limited to non-clinical educational and scientific guidance to support the accuracy and responsible presentation of general information; it does not provide patient care and does not participate in prescribing or individual treatment decisions. Any diagnosis, treatment, or prescription decisions are made solely by independent, U.S.-licensed healthcare professionals as part of a telehealth consultation and subject to applicable law and required consents.
THE SERVICE IS PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS. ACCESS TO AND USE OF THE PLATFORM IS ENTIRELY VOLUNTARY AND AT YOUR OWN RISK. TO THE FULLEST EXTENT PERMITTED BY APPLICABLE LAW, US QUICK HEALTH LLC AND ALL THIRD PARTIES PROVIDING PRODUCTS OR SERVICES THROUGH THE PLATFORM, INCLUDING AFFILIATED MEDICAL GROUPS, HEALTHCARE PROVIDERS, PHARMACIES, AND LABORATORIES, DISCLAIM ALL WARRANTIES, REPRESENTATIONS, AND CONDITIONS OF ANY KIND, WHETHER EXPRESS, IMPLIED, STATUTORY, OR OTHERWISE. THIS INCLUDES, WITHOUT LIMITATION, ANY IMPLIED WARRANTIES OF MERCHANTABILITY, NON-INFRINGEMENT, SATISFACTORY QUALITY, AND FITNESS FOR A PARTICULAR PURPOSE, AS WELL AS ANY WARRANTIES ARISING FROM COURSE OF DEALING OR USAGE OF TRADE.
US Quick Health LLC makes no warranty or representation that the Service or any information, content, products, services, merchandise, or other materials provided through the Platform will be accurate, complete, reliable, timely, or useful. We do not guarantee that the Platform will operate without delays, interruptions, defects, security breaches, viruses, malware, cyberattacks, or other harmful events or imperfections.
TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, UNDER NO CIRCUMSTANCES SHALL US QUICK HEALTH LLC BE LIABLE TO YOU OR TO ANY OTHER PERSON OR ENTITY FOR ANY INDIRECT, INCIDENTAL, SPECIAL, EXEMPLARY, OR CONSEQUENTIAL DAMAGES, INCLUDING BUT NOT LIMITED TO PERSONAL OR BODILY INJURY, EMOTIONAL DISTRESS, WRONGFUL DEATH, LOSS OF DATA, LOST PROFITS, BUSINESS INTERRUPTION, OR DAMAGES ARISING FROM YOUR ACCESS TO OR USE OF, OR INABILITY TO USE, THE SERVICE. THIS INCLUDES ANY INFORMATION OR CONTENT MADE AVAILABLE THROUGH THE PLATFORM OR ANY SERVICES PROVIDED OR PRODUCTS SUPPLIED BY THIRD PARTIES. SUCH LIMITATIONS APPLY REGARDLESS OF WHETHER THE CLAIM IS BASED IN CONTRACT, WARRANTY, TORT (INCLUDING NEGLIGENCE), STRICT LIABILITY, OR ANY OTHER LEGAL THEORY, AND WHETHER OR NOT US QUICK HEALTH LLC HAS BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.
US Quick Health LLC’s total liability for any claims, damages, or losses relating to or arising out of your use of the Service shall be limited solely to the actual amount of damages you incur, not to exceed one thousand U.S. dollars (USD $1,000).
Any claim or cause of action arising from or related to your use of the Service must be filed within one (1) year of the event giving rise to such claim. Claims not brought within this period shall be permanently barred.
Certain jurisdictions may not permit the exclusion of certain warranties or the limitation or exclusion of liability for incidental or consequential damages. In such cases, any liability that cannot legally be disclaimed or limited shall be restricted to the narrowest scope and shortest duration permitted under applicable law.
You agree to defend, indemnify, and hold harmless US Quick Health LLC, its affiliates, subsidiaries, directors, officers, employees, contractors, and agents, as well as any Third Parties offering products or services through the Platform, including medical groups, healthcare providers, pharmacies, and laboratories, from and against any and all claims, demands, actions, suits, proceedings, damages, liabilities, losses, costs, expenses, and fees (including reasonable attorneys’ fees and litigation expenses) that arise out of or relate to: (i) your use or misuse of the Service; (ii) your violation of these Terms; (iii) your violation of applicable law or regulation; (iv) your fraud, willful misconduct, or gross negligence; or (v) your infringement or violation of the rights of any other individual or entity.
US Quick Health LLC reserves the right to assume the exclusive defense and control of any matter for which it is entitled to indemnification, and you agree to cooperate fully and reasonably with any such defense at your expense.
Any notices or communications from US Quick Health LLC regarding the Service or these Terms may, at our sole discretion, be delivered to you by email, by posting a notice within the Platform, or by regular mail to the address associated with your account.
When you access or use the Platform, or when you send emails or SMS messages to US Quick Health LLC, affiliated medical groups, or their providers, you are communicating electronically. By using the Service, you consent to receive communications from us, from the medical groups, and from their providers in electronic form. Such communications may be delivered by email, SMS message, or through features within the Platform. You agree that all agreements, disclosures, notices, and other communications that we provide to you electronically satisfy any legal requirement that such communications be made in writing. You further agree that any notices transmitted electronically by US Quick Health LLC will be deemed to have been received on the date they are sent, as outlined in these Terms. Test.
It is the policy of US Quick Health LLC to terminate access to the Service by any user who repeatedly infringes copyright, once proper notification has been provided to us by the copyright owner or the copyright owner’s authorized legal representative.
If you believe that your work has been copied, posted, or otherwise used on the Platform in a manner that constitutes copyright infringement, you must submit a written notice to our designated Copyright Agent that includes the following information: (1) your physical or electronic signature, or that of the person authorized to act on behalf of the copyright owner; (2) a description of the copyrighted work you claim has been infringed; (3) identification of the material on the Platform that you allege is infringing and its location; (4) your address, telephone number, and email address; (5) a statement by you, made in good faith, that the disputed use is not authorized by the copyright owner, its agent, or the law; and (6) a statement, made under penalty of perjury, that the information in your notice is accurate and that you are the copyright owner or authorized to act on the copyright owner’s behalf.

You and US Quick Health LLC agree that any legal claim, dispute, or controversy between you and us, or between you and any affiliated medical groups, providers, laboratories, or pharmacies, arising out of or relating in any way to the Platform, the Service, the Content, or any related products, services, or advertising (collectively, “Disputes”), that cannot be resolved through an informal resolution process, shall be settled through confidential and binding arbitration before a single commercial arbitrator from National Arbitration & Mediation (“NAM”), rather than in court. The arbitration will be governed by NAM’s Comprehensive Dispute Resolution Rules and Procedures and, where applicable, its Supplemental Rules for Mass Arbitration Filings (together, the “Rules”). By agreeing to these Terms, both you and US Quick Health LLC knowingly waive the right to a trial by jury and to pursue claims in state or federal court, except where expressly provided herein. This Arbitration Agreement applies to all Disputes, including those that may have arisen prior to acceptance of these Terms or that arise after termination of these Terms.
Before arbitration may be commenced, both parties agree to first engage in a good faith effort to resolve any Dispute through an informal conference (“Informal Dispute Resolution Conference”). Such a conference will be held telephonically or via videoconference within forty-five (45) days of written notice of the Dispute, unless otherwise agreed. The notice must include your full name, contact information, and a description of the Dispute, and must be signed by you personally. If you are represented by counsel, your counsel may participate in the conference, but you must also participate directly. During this process, applicable statutes of limitations and filing deadlines will be tolled.
If the Dispute cannot be resolved within thirty (30) days of the Informal Dispute Resolution Conference, either party may file a request for arbitration. A written arbitration request must include your name, residence address, contact information, a detailed description of the Dispute, the relief sought, and certification that the Informal Dispute Resolution Conference was completed. For claims against US Quick Health LLC, such notices must be sent by email to [email protected] or by mail to US Quick Health LLC, Attn: Dispute Notice, US QUICK HEALTH, LLC, 7345 W SAND LAKE RD STE 210 – OFFICE 2630, ORLANDO, FL 32819.
Arbitration will be conducted in Wyoming or in the county of your residence, unless otherwise agreed in writing or required by the batch arbitration rules. The arbitrator’s decision shall be final and binding, and judgment on the award may be entered in any court of competent jurisdiction. This Arbitration Agreement is governed by the Federal Arbitration Act, 9 U.S.C. § 1 et seq.
Payment of arbitration fees will follow the applicable NAM fee schedules. If the arbitrator determines that a claim was filed in bad faith or for an improper purpose under the standards of Federal Rule of Civil Procedure 11(b), the arbitrator may award fees and costs to the other party. Each party will otherwise bear its own attorneys’ fees unless governing law provides otherwise.
Notwithstanding the foregoing, either party may bring an individual claim in small claims court within the scope of that court’s jurisdiction instead of arbitration. In addition, claims involving intellectual property rights, unauthorized use, piracy, theft, or violations of the Computer Fraud and Abuse Act may be brought directly in a state or federal court in Wyoming, and both parties consent to the personal jurisdiction of such courts for those claims.
YOU AND US QUICK HEALTH LLC AGREE THAT ALL DISPUTES SUBJECT TO THIS ARBITRATION AGREEMENT MUST BE CONDUCTED ON AN INDIVIDUAL BASIS ONLY. NO CLAIM MAY BE BROUGHT AS A CLASS ACTION, COLLECTIVE ACTION, OR REPRESENTATIVE ACTION, NOR MAY CLAIMS BE CONSOLIDATED EXCEPT AS EXPRESSLY PERMITTED UNDER NAM’S BATCH ARBITRATION RULES. The arbitrator may award relief only to the individual party bringing the claim and only to the extent necessary to provide relief for that individual’s claims. If a final court decision determines that any part of this class waiver cannot be enforced with respect to a claim, then that specific claim may proceed in court, while all other claims remain subject to arbitration.
To promote efficiency, in the event that twenty-five (25) or more individual arbitration claims of a substantially similar nature are filed against us by or with the assistance of the same law firm or group of firms within ninety (90) days, NAM will administer those claims in batches of up to one hundred (100) arbitrations per batch. A single arbitrator will be appointed for each batch, and the batch will proceed under one schedule, with one set of fees and one hearing (if any). This “Batch Arbitration” process is designed solely to streamline administration and does not authorize or create a class or collective arbitration. If the Batch Arbitration provision is found unenforceable, the entire Arbitration Agreement shall be void, and all claims shall instead be heard exclusively in the state or federal courts located in Wyoming.
If you do not wish to be bound by this Arbitration Agreement, you may opt out within thirty (30) days of first becoming subject to it. To opt out, you must send a signed written notice including your name, address, email address, and a clear statement that you wish to opt out of the Arbitration Agreement to US Quick Health LLC, Attn: Dispute Notice, US QUICK HEALTH, LLC, 7345 W SAND LAKE RD STE 210 – OFFICE 2630, ORLANDO, FL 32819. Opt-outs must be submitted individually and may not be submitted on behalf of others.
If we make material changes to this Arbitration Agreement in the future, such changes will not apply to any Dispute for which a formal arbitration request has already been submitted. Continued use of the Service following any such modification constitutes acceptance of the updated terms, except where prohibited by law.
Entire Agreement
These Terms, together with any additional agreements or policies that US Quick Health LLC may post on the Platform or that you and US Quick Health LLC may enter into from time to time, constitute the entire agreement between you and US Quick Health LLC with respect to your use of the Service. They supersede and replace any prior agreements, understandings, or representations between you and US Quick Health LLC relating to the Service, including any previous versions of these Terms.
This Service is operated by US Quick Health LLC from its offices in the State of Wyoming. Individuals who choose to access the Platform from locations outside the United States, or from jurisdictions where use of the Service or its content is restricted or unlawful, do so at their own initiative and are solely responsible for complying with applicable local laws. Access to the Service from territories where its content is prohibited is expressly forbidden.
The validity, interpretation, and enforcement of these Terms shall be governed by and construed in accordance with the laws of the State of Wyoming, without regard to conflict of law principles.
All provisions of this Agreement shall apply to the fullest extent permitted by law. The failure of US Quick Health LLC to enforce any particular provision shall not constitute a waiver of that right or of any other rights under these Terms. If any provision of this Agreement is found to be invalid, illegal, or unenforceable, that provision shall be replaced by terms that most closely reflect the original intent of the parties, to the extent permitted by law, and the remaining provisions shall continue in full force and effect. Section headings are provided for convenience only and shall not affect the meaning or interpretation of these Terms.
Nothing in these Terms, nor any use of the Service, the Platform, or the Content, shall be construed as creating a partnership, joint venture, employment relationship, or agency of any kind between you and US Quick Health LLC, or between you and any affiliated medical groups, providers, laboratories, or pharmacies. You are not authorized to, and may not, enter into contracts on behalf of US Quick Health LLC or bind us in any way.
You may not assign, transfer, or delegate any of your rights or obligations under this Agreement without the prior written consent of US Quick Health LLC, and any such attempted assignment shall be null and void. US Quick Health LLC may, at its sole discretion and without the need for additional consent, assign or transfer this Agreement, in whole or in part, together with any rights or obligations under it, to any of its affiliates or to a third party in connection with a merger, acquisition, sale of assets, corporate restructuring, or other transfer of business.
Any use of third-party software made available in connection with the Service, or any Third-Party Goods and Services accessed through the Platform, will be subject to the applicable license terms or terms of use of such third party. Where no such license or terms exist, your use will be governed solely by this Agreement. In addition, your use of the Service must comply with all applicable third-party agreements, if any.
Except as expressly provided in this Agreement — including provisions concerning indemnification in favor of US Quick Health LLC, affiliated medical groups, providers, pharmacies, and laboratories, as well as the arbitration agreement — nothing in these Terms is intended to create or confer third-party beneficiary rights. No individual, subcontractor, or third party shall have the right to enforce any provision of this Agreement or to claim any benefit under it, unless explicitly stated herein.
