Important Safety Information — Wegovy® (semaglutide) Injection

Warning: Risk of Thyroid C-Cell Tumors

In studies with rodents, semaglutide — the active ingredient in Wegovy — caused thyroid C-cell tumors, including thyroid cancer. It is not known whether Wegovy causes thyroid tumors, including medullary thyroid carcinoma (MTC), in humans.

Tell your healthcare provider right away if you notice a lump or swelling in your neck, persistent hoarseness, difficulty swallowing, or shortness of breath. These may be signs of thyroid cancer.

Do not use Wegovy if you or a family member has ever had medullary thyroid carcinoma (MTC) or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).

FDA-Approved Uses

Wegovy (semaglutide) is approved by the U.S. Food and Drug Administration (FDA) for:

  • Chronic weight management in adults with obesity or overweight in combination with a reduced-calorie diet and increased physical activity.
  • Reducing the risk of major cardiovascular events (heart attack, stroke, or cardiovascular death) in adults with known cardiovascular disease and either obesity or overweight.

Limitations of Use:

  • Wegovy should not be used together with other semaglutide-containing products or other GLP-1 receptor agonists.

Who Should Not Use Wegovy

Do not use Wegovy if:

  • You or any family member has ever had medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
  • You have a known allergy to semaglutide or any component of the product.

How to Use Wegovy

Wegovy is administered once weekly as a subcutaneous injection in the abdomen, thigh, or upper arm.
Rotate injection sites each week. It may be taken with or without food.

Use Wegovy on the same day each week, at any time of day.
Do not adjust your dose or discontinue the medication without first consulting your healthcare provider.

Your provider may guide you through gradual dose increases every four weeks to minimize gastrointestinal side effects.

Before Using Wegovy — Tell Your Healthcare Provider

Inform your provider about all medications, including prescription and over-the-counter drugs, vitamins, and supplements.

Important interactions include:

  • Diabetes medications: Insulin or sulfonylureas (e.g., glipizide, glimepiride) may increase the risk of low blood sugar.
  • Other GLP-1 medications: Do not use Wegovy with products such as Ozempic, Saxenda, Victoza, Byetta, or Bydureon.
  • Oral medications: Wegovy delays gastric emptying and may alter absorption of oral drugs.
  • Weight-loss products or supplements: Avoid combining with other weight-management medications.

Tell your provider if you have or have had:

  • Type 1 or type 2 diabetes
  • Thyroid cancer or thyroid nodules
  • Pancreatitis
  • Kidney disease or liver disease
  • Diabetic retinopathy
  • Depression or suicidal thoughts

Pregnancy and breastfeeding:

  • Wegovy should not be used during pregnancy. Weight loss during pregnancy offers no benefit and may harm the unborn baby.
  • Discontinue Wegovy at least 2 months before a planned pregnancy, since semaglutide may remain in the body for an extended time.
  • It is unknown whether Wegovy passes into human breast milk. Discuss with your provider before breastfeeding.

Serious Side Effects — Seek Immediate Medical Attention If:

Call 911 or go to the nearest emergency department if you experience:

  • Thyroid C-cell tumors: Lump in the neck, hoarseness, difficulty swallowing, or shortness of breath.
  • Pancreatitis: Severe, persistent abdominal pain (may radiate to the back) with or without vomiting.
  • Gallbladder disease: Upper abdominal pain, jaundice (yellow skin or eyes), fever, or pale stools.
  • Low blood sugar (hypoglycemia): Shakiness, sweating, dizziness, confusion, or rapid heartbeat — especially when used with insulin or sulfonylureas.
  • Kidney injury or dehydration: Caused by severe nausea, vomiting, or diarrhea. Stay hydrated.
  • Severe gastrointestinal reactions: Persistent or severe nausea, vomiting, or constipation.
  • Severe allergic reaction: Swelling of the face, lips, tongue, or throat; rash or itching; trouble breathing; or fainting.
  • Vision changes: Especially if you have diabetic retinopathy.
  • Increased heart rate: Report sustained or unexplained racing heartbeat.
  • Mood changes or suicidal thoughts: Contact your provider immediately if you experience new or worsening mental-health symptoms.
  • Pulmonary aspiration during anesthesia: Inform your provider if you are scheduled for surgery requiring sedation or general anesthesia, as delayed gastric emptying may increase aspiration risk.

Never share your injection pen with others — sharing can spread infection.

 

Common Side Effects

The most frequently reported side effects of Wegovy include:

  • Nausea
  • Vomiting
  • Diarrhea
  • Constipation
  • Abdominal pain or discomfort
  • Headache
  • Fatigue
  • Indigestion (dyspepsia)
  • Dizziness
  • Bloating (abdominal distension)
  • Belching
  • Gas (flatulence)
  • Heartburn (GERD)
  • Stomach infection (gastroenteritis)
  • Low blood sugar in patients with type 2 diabetes

Most side effects are mild to moderate and tend to improve over time. Contact your healthcare provider if symptoms persist.
